Physical facilities not maintained in good repair. Repair the damage wall under the right side of wok line and left side of warewashing machine area. Floors and walls shall be smooth, durable and easily cleanable. Provide a proper coved base finish.

Refrigerated, ready-to-eat, TCS foods not properly date marked. Multiple foods (cooked meats, cooked fish, open tofu, etc) were not properly date marked as required. Date marking and labeling needs improvement.

Non-food contact surface(s) not cleaned at the required frequency. Observed accumulation of food debris and residues on doors to walk-in and reach-in cold holding equipment.

Light intensity less than fifty foot candles in required areas. Observed measured light intensity to be less then 50 foot candles where employees work with knives and slicers at the time of inspection

Physical facilities not maintained in good repair. Observed ceiling over the dirty plate cart to be missing or has damaged ceiling tiles. Repair the cause of the water leak and replace the ceiling tiles.

TCS foods not being cold held at the proper temperature. Observed multiple cooked chicken items (cooling), cut cabbage, raw chicken, were not holding at acceptable cold temperatures.

Presence of live insects. Observed live insects in monitoring traps under the dish machine and live pest were observed under the wok line (drip tray). Manager stated that Pest Control Operator has been scheduled for early next week. Cleaning needs to improve and unnecessary items need removed to reduce pest harborage.

Unnecessary or nonfunctional items and /or litter on premises. Observed an unused fish tank. Remove fish tank. Observed multiple equipment in bar and food prep that are no longer used. Remove all unnecessary items from the FSO.

Unnecessary single use items through out the Food Prep area, in private storage room, around the bar and employee area. Remove all unneeded items from the FSO to reduce clutter and pest harborage.

PIC unable to demonstrate knowledge by having no critical violations.
Comments
Observed the cleaning has improved. Pest control operator will now be on a weekly schedule to monitor and treat the pest issue. No live pests were observed at the time of inspection. Pest harborage issues has improved. Repairs are still need to the floors and walls in the kitchen.
Based on this follow-up inspection, the operation has made significant improvements to be compliant. A follow-up inspection has be scheduled for 10/14 to ensure these efforts have been maintained.

Due to the amount of repeat and critical violations, this FSO needs to maintain compliance. Due to the long history of the FSO failing to comply with the Food Code, the FSO will need to continue to demonstrate significant efforts to improve to avoid additional enforcement action from the Licensor.

Notes:
1) Door handles to reach-in coolers and freezers shall be maintained in good repair.
2) Install a door handle on storage room door. Recommend painting door and walls in this area when able.
