3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Observed container of chicken soup (7/19), sausage gravy (7/14), Turkey (7/18), Ham (no date), fruit salad (no date), lettuce (no date) in the walk-in and reach-in refrigeration unit past date marks. PIC voluntarily discarded the products.

Facility does not have an employee with manager certification in food protection.

3717-1-03 / Food - safe, unadulterated, and honestly presented
Corrected During Inspection Food was unsafe, adulterated, or not honestly presented. Observed bag of spinach with unknown dark liquid throughout the interior and exterior. PIC voluntarily discarded product.

3717-1-06.1(K) / Insect control devices - design and installation.
Improper use or placement of insect control devices. Observed Hot Shot vapona strip under the prep sink, at the warewashing machine, and in the dining area. Please ensure label is being followed for usage.

3717-1-02.2(G) / Jewelry - prohibition.
Food employees wearing jewelry on arms or hands during food preparation.

3717-1-04.8(F) / Storage - prohibitions - single-use/single-service articles.
Observed boxes of single-use items and dry stock stored on the floor in the dry storage room.

3717-1-03.2(N)(2) / Gloves - slash resistant gloves
Slash resistant gloves used improperly. Observed no single-use glove being used over the cut gloves to prepare ready-to-eat food. Discussed glove usage with employee.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed the following non-food contact surfaces of equipment with an accumulation of food residue or debris: 1) can opener. 2) knife holder. 3) air drying rack/shelving. 4) steamer equipment leaking water.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Repeat Plumbing system not properly maintained or repaired. Observed damaged atmospheric vacuum breaker for the water line at the warewashing machine and garbage disposal.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors near the steamer, in the warewashing area, and at the coffee/prep area not maintained cleaned. Ensure covebase is installed at all floor/wall junctures.

Complaint on 26-Jul-2021 :
An inspection was conducted in regards to a complaint. A standard/ccp inspection was also conducted.
Per complainant: Worried that the operation is going to get people sick. They are storing raw ground beef above lettuce and other products. The beef looks horrible and blood/juice is all over the walk-in cooler.
Complaint findings: Observed ground beef thawing on the bottom rack in the walk-in cold holding unit. The product was thawing in a container to collect any liquids that are produced.
Complaint is now considered closed.

The violations documented shall be corrected and will be checked during the follow-up inspection.
Comments related to the Critical Control Point Inspection (Item 35) are listed below. Note: Positive observations are indicated by the element number and the letter P.
