3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Chlorine sanitizing solution at incorrect concentration. Observed below 25 ppm of chlorine measured at the chemical mechanical warewashing equipment. PIC changed the sanitizer bucket and primed the line to ensure proper concentration is being provided (25-100 ppm). PIC also contacted company to service equipment to ensure equipment is functioning correctly.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Observed containers of tcs foods past date mark in the walk-in. PIC voluntarily discarded the products.

3717-1-04.4(B) / Cutting surfaces.
Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed discolored prep/cutting board at the main preparation refrigeration equipment.

3717-1-04.4(P) / Warewashing equipment - determining chemical sanitizer concentration.
Sanitizer test kit or other device not being used.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of food residue or debris on nonfood contacts surfaces of the refrigeration units which include the door handles, doors, gaskets, and door lids throughout the facility. Ensure gaskets that are damaged are being repaired/replaced (main prep cooler and under serving window cooler). .

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Corrected During Inspection Observed water leaking below the atmospheric backflow prevention device above the chemical mechanical warewashing equipment (leak above the dish machine). PIC contacted company for service of dish machine and was also able to repair leak).

3717-1-06.1(R) / Outdoor refuse areas - curbed and graded to drain.
Repeat Outdoor refuse areas not curbed and graded to drain to collect and dispose of liquid waste that results from the refuse and from cleaning the area and waste receptacles.

3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Walls and/or ceilings not smooth and easily cleanable. Observed damaged walls near the service window, under the microwaves, near the handsink by the grill.

3701-21-03 / Facility layout and equipment specifications for FSO
Repeat FSO did not submit facility layout and equipment specifications. Observed renovations to the facility without submitting plans and going through facility review for the new bar area that services the outdoor patio space. PIC stated the flooring in the facility may be altered, please include this in the facility review packet and submit prior to construction.
