Comments
MFSO was not in operation at the time of inspection. Inspection was completed with Kent Fire Prevention present. Hot water was observed at the 3 compartment sink and hand washing sink. The MFSO owner claims all foods are will be prepared on the MFSO and foods are purchased as needed per events.

Notes:
1) Ensure the potable water tank has been properly cleaned and sanitized before use.
2) Recommend providing documentation regarding the warehouse/processor location that has been approved by ODA. Foods that are made or stored in a residential home are to be considered from and unapproved source and should be embargoed.
3) Repair flooring when tiles are not holding when able.
4) Current reach-in cooler is approved until replacement is needed. Equipment will need to be a NSF or equivalent testing agency approved unit when able.

A signed application and payment for the amount of $330 (Check#1164) was received at the time of inspection.
