3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. The current employee health reporting agreement does not include all employees. PIC will ensure emplyees working today sign the policy and stated they will ensure all employees that are not at work today sign the policy.

3717-1-03.4(A) / Frozen food - temperature and time control.
Foods in frozen storage exhibit signs of thawing or refreezing. Observed table-top reach-in freezer accross from prep line overfilled and prevent equipment from keeping product frozen. Continue to monitor this equipment, half the products were removed.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed ice build-up in the walk-in freezer and on the condensation line.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors under and around equipment not maintained clean. Damaged areas shall be repaired to allow for easy cleaning.

Ensure employees are consistent with date marking, prep date counts as day 1 (add 6 to current date for 7 day date marking).
Untreated wood shelving is difficult to clean and shall not be used in the facility.
The violations documented shall be corrected and will be checked during next standard inspection.
