Comments
In order for the Facility Review to be approved the following must be completed:
1) Provide a current Facility Site Plan
2) Provide a list of all the make and model numbers for all the equipment present or planned to be used at the facility.
In order for a Retail Food Establishment License to be issues the fooling must be completed:
1) Repair the water damage dry wall in the back rest-room. Unknown black substance has grown on the wall.
2) Back door must be self closing and self latching.
3) Remove all equipment that is not necessary to the every day operation of the RFE from the premises. All coolers, stoves, containers, warmers, etc. shall be removed before license will be issued.
4) Remove all the unnecessary debris, trash, plastic containers, old equipment from the exterior of the facility. The outside dumpster area must be free of trash and debris.
5) Remove the non-commercial reach-in cooler and freezer (Maytag). This cold holding unit will not be approved use in the RFE.
6) Provide a proper air gap on the both drainage pipes from the ice machine. Air gap must be at least 2 inches.
7) Clean the interior, exterior and floor under of the ice machine. Recommend having it professional serviced.
8) Provide a smooth, durable and easily cleanable surface on walls behind and around the mop/service sink. Recommend using FRP, tile, or similar material. Painted dry wall will not be approved.
9) Clean all walls, floors, doors, and condensation vents in the walk-in cold holding unit.
10) Repair the leaking condensation pipes.
11) Clean all air vents throughout the retail area of the facility. Clean the the rafters and ventilation. This is a pre-existing violation.
12) Provide proof of level 1 Person In Charge food safety training for at least one individual responsible for managing RFE.
