Comments
This inspection conducted per the request of the owner to approve the new trailer has been purchased and constructed to replace the knockdown MFSO that was originally licensed. A new layout with make and model numbers have been provided. The MFSO unit is approved to be used with the understanding that the following will be provided/installed before operating:
1) Provide a proper sign on the exterior of the MFSO with the Name, city of origin, and phone number. (in the process)
2) Provide a "Employees Must Wash Hands" sign at the designated hand sink. (in the process)
3) Recommend providing a proper vomit/diarrheal clean-up procedure and kit. (in the process)
4) Provide verification that the manager has trained all present employees on the symptoms and diseases that are required to reported. Documentation of the training shall be provided for review.
5) Recommend provided PIC training for each manager per shift. (completed)
6) Owners is in the process of obtaining a warehouse license from ODA. Recommend contacting a proper kitchen that can be used as a commissary (provide documentation).
7) Post a Choking poster (completed)
8) Post a no smoking sign (completed).
9) Ensure the backflow prevention device ASSE standard can be seen. (completed)
10) A proper test kit is required when operating. (completed)

Restriction: until a commissary kitchen can be utilized, the cucumbers and tomatoes for the pasta salad shall be purchased prewashed or ready to eat. If washing on the MFSO is necessary, provide a colander or food grade container to use with in the sanitize sink basin. The sinks shall be washed rinsed and sanitized before each use.


Owner stated that the original license was never received and has requested a reprint of the license. At this time a reprint of the license will be provided (last one for the year) and a new layout will be be included on the back.
