| This Retail Food Establishment (RFE) has undergone a final inspection and has been approved. A Certificate of Occupancy has been received Proof of Person-In-Charge Training was received and accepted. Recommend contacting Ohio Department of Health to make sure the training is approved and meets their standards (per the new code requirements). A signed application and payment for the amount of $213 has received for license # JSMH-9ZHJHF at the time of inspection. Operator will be issued a 2016 risk class level I - 15 month license for the cost of a 2015 license because the 2016 license fees have not been finalized by the Board of Health. Only Retail commercial pre-package foods are to be sold at this location. Any preparation of foods will require a increase in Risk Level Classification. Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. Okay to operate |
