3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Repeat Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed the FSO without signed employee reporting agreements at time of inspection.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed food-contact surfaces on unapproved saw in the back room with an accumulation of food debris at the time of inspection.
To prevent contamination, equipment food-contact surfaces shall be clean to sight and touch. Unapproved saw shall be removed from the facility. An approved meat saw shall be used for cutting meats.

3717-1-03.1(A)(3) / Sources - packaged food labeling
Repeat Observed packaged meats from the licensed kitchen next door called Szechuan Gourmet (same owner) were not properly labeled at the time of inspection. Items did not contain the type of meat/cut of meat and did not contain the product of origin.
To properly inform consumers, packaged food shall be labeled as specified in law. Provide proper labeling for the packaged foods recieved from Szechuan Gourmet. Contact the Health Department if additional information is needed.
3717-1-03.5(C)(3) / Food labels - bulk food
Repeat Observed bulk foods on display for customer self-service that were not properly labeled at the time of inspection. Signs only contained the chinese wording for the products.
To properly inform customers, bulk food that is available for consumer self-dispensing shall be prominently labeled with the information specified under (C)(2) of this rule. Provide proper identification on the signs. Contact the Health Department if additional information is needed.

901:3-4-7.0/Facility layout and equipment specifications for RFE
An RFE was constructed, altered, and/or equipment was added without prior approval from the licensor. Observed a gas household stove/oven unit installed in the back warewashng room at the time of inspection. Observed the gas household stove removed from the facility at the time of inspection.
To prevent foodborne illness, no person, firm, association, organization, corporation, or government operation shall construct, install, provide, equip, or extensively alter a retail food establishment until the facility layout and equipment specifications therefore have been submitted to and approved in writing by the licensor. When the facility layout and equipment specifications are submitted to the licensor, they shall be acted upon within thirty calendar days after date of receipt. The facility layout and equipment specifications shall be approved by the licensor in accordance with the rules adopted pursuant to section 3717.05 of the Revised Code stating the criteria for facility layout and equipment specifications.
Comments
Kent food Market will become a risk level 3 for next licensing year due to packaged meat operation that the facility is conducting.
Meats and vegetables are being packaged in the Kent food market facility.
Bulk meats are being cut in the licensed restaurant next door called Szechuan Gourmet (same owner).

The violations documented on this report will be checked during the re-inspection scheduled for Monday, April 24, 2017.
