3717-1-04.1(DD) / Warewashing machines - automatic dispensing of detergents and sanitizers.
Critical Repeat Warewashing machine does not automatically dispense detergent sanitizer and/or does not have a visual means to ensure detergent and sanitizer is automatically dispensed. PIC developed a log to help montior the chemcals used in the dish machine.
3717-1-02.4(A)(2) / PIC - Manager Certification
Facility does not have an employee with manager certification in food protection. PIC is enrolled in a course to obtain the Ohio Department of Health (ODH) manager level certification.
3717-1-06.4(A) / Repairing.
Damaged floor and wall junctures near the prep sink and in the warewashing area. Observed damaged wall surfaces throughout (specifically near dry storage doorway). PIC states maintenance will be repairing damaged wall and possible painting. Ensure work is being done when food prep is not occuring. Observed the drain line under the prep sink in need of adjustment.

| Observed the facility to be clean and organized. PIC demonstrated excellent knowledge in food safety within the operation. Floor drain under the dish machine was being serviced due to a back-up. PIC stated it will be properly cleaned up when service is complete. The violations documented are to be corrected and will be checked during next standard inspection. |
