Critical Food not properly protected from contamination by separation, packaging, and segregation. Raw turkey was stored over beef in the walk in cold holding unit. Education was provided about proper storage.

Repeat Food employees not wearing proper hair restraint.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment not approved by a recognized testing agency (non-commercial rice cooker).

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean (cold holding equipment, fryer, exhaust hoods, storage racks and shelving).

Repeat Physical facilities not maintained in good repair. Kitchen ceiling tiles are damaged. Men's restroom sink is without cold water. The basement is in disrepair.
