| This RFE has undergone a final facility review pre-licensing inspection and has been approved to be licensed under the following conditions. 1) Provide a proper air gap (2 inch) on the drainage lines from the coke beverage machine the coke machine ice bin. 2) Ice machine must be properly cleaned before ice is to be used for customer beverages. 3) Clean the dust accumulation on condenser unit in the walk-in cooler. 4) Repair the damaged cabinet that the coke beverage machine and slush machine is installed on. The back wall molding is damaged and separating from the wall. 5) Install FRP on the wall behind the coke and slush machine to provide a smooth, durable and easily cleanable surface. These items will be re-check at the 30 day inspection. If not completed they will be documented as violations. A completed signed application and check # 3009 for the amount of $219 was received at the time of inspection. This RFE has been approved to operate as a Risk Class II. Okay to operate! |
