Comments
In order for a Risk Level I Retail Food Establishment license to be issued to this location, the following must be completed:

1) Provide a signed letter by the owner that the current facilities will be replaced with a brand new building or will be remodeled with in a year. Please provide a deadline date when the construction is planned to begin. If the document is unable to be provided, then all items on this list must be completed before a application for a license will be provided.
2) Repair or replace the floor tiles in back half of the retail display area. Multiple floor tiles are missing or damaged.
3) Repair the transition molding where tiles type changes
4) Provide a Service Sink for proper disposal of mop water. Until this sink is provided, mop water may disposed in the toilet. Do not dump mop water outside.
5) Repair damaged or missing ceiling tiles.

The following items shall be corrected regardless of the future plans of building:
6) Provide a properly located and functioning temperature measuring device with in the Deli Express cold holding equipment.
7) Provide documentation from the supplier regarding the shelf life if the Deli express products and any other TCS food that will be held for longer than 7 days. Manager stated that the sandwich products from deli express are dated for a 14 day discard and the other brand they carry is 21 days.
8) Provide the make and model number for the new coffee equipment. If specs are available, please provide them. Since the facility is not equipment with a 3 compartment sink, only equipment that will not need ware washing (wash, rinse, and sanitize) will be approved.
9) Repair the hand washing sink in the restroom. Provide paper towels and trash can with lid.
10) Install a mop hanger in the restroom for proper mop storage.
11) Install a outlet switch cover in restroom.
12) Remove old coffee equipment, with out the means to ware wash, this equipment shall not be present.
13) Clean all beer/beverage refrigerators interior and sliders. Clean shelves as needed.
14) Repair/seal all holes in storage shed. Prevent rodent entry into the shed storage.

Once the items listed above have been completed, contact the Kent City Health Department to schedule a final pre-licensing inspection. This needs to happen as soon as possible as the RFE is currently operating without a license. An application for a RFE license will be provided once item 1 and 6-14 are completed.
