3717-1-05.1(J) / Backflow prevention device - when required.
Critical Observed no approved backflow prevention device on the ice machine's water connection located in the basement at the time of inspection.
To prevent contamination, a plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the RFE, including on a hose bibb if a hose is attached or on a hose bibb if a hose is not attached and backflow prevention is required by the Ohio building code, by: (1) Providing an air gap as specified under paragraph (D) of this rule; or (2) Installing an approved backflow prevention device as specified under paragraph (E) of this rule. Provide a backflow prevention device for the ice machine's water supply.
3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from the ice bin behind the bar and the ice machine in the basement in which food, portable equipment, or utensils are placed at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Provide air gaps for the ice bin and the ice machine to meet the requirements of this rule

3717-1-03.5(C)(2) / Food labels - packaged food
Observed labels on food packaged (Sugar Luv candy/desserts) in the RFE that did not contain all required information.
To prevent health hazards, packaged foods shall be labeled as specified in this rule. Food label information shall include: (a) The common name of the food, or an adequately descriptive identity statement; (b) If made from two or more ingredients, a list of ingredients in descending order of predominance by weight, including a declaration of artificial color or flavor and chemical preservatives, if contained in the food; (c) An accurate declaration of the quantity of contents; (d) The name and place of business of the manufacturer, packer, or distributor; (e) The name of the food source for each major food allergen contained in the food, unless listed in the ingredients; (f) Nutritional information when required by law.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed single-use articles (drink stirs on the bar) that were not properly stored at the time of inspection.
To prevent contamination, single-use articles shall be stored as specified under paragraph (E)(1) of this rule and shall be kept in the original protective package or stored by using other means that afford protection from contamination until used. Provide proper protection for the drink stirs to meet the requirements of this rule.
3717-1-06.1(M) / Outer openings - protected.
Observed the basement door without proper protection (gap under the door) against the entry of insects and rodents at the time of inspection.
To prevent health hazards, outer openings of an RFE shall be protected against the entry of insects and rodents by (a) Filling or closing holes and other gaps along floors, walls, and ceilings; (b) Closed, tight-fitting windows; and (c) Solid, self-closing, tight-fitting doors. The gap below the basement door shall be repaired to meet the requirements of this rule.

A handwashing sink may not be used for purposes other than handwashing.
