Comments
This inspection was completed per the request of the operator to approve the mobile food trailer to be issued a MFSO license. A signed applications and License fee for the amount of $310 (Check# 2937) was received at the time of inspection. The unit was not in operation at the time of inspection. Observed cold holding equipment to be functioning correctly at the time of inspection. Hand washing sinks and 3 compartment sinks were observed to be operational and complainant.

Okay to Operate with the understanding the following will be addressed:

1) Clean and resurface the flat top grill. Must be completed before operating.
2) Repair or replace leaking water heater. Ensure proper hot water temperatures can be provided for hand washing and warewashing.
3) Redirect the waste water from cold and hot holding to grey water storage.
4) Resurface or replace flooring where rusting.
5) Provide a completed commissary agreement (provided with inspection email) form with details regarding the Ohio Department of Agriculture Processing or Warehouse Registration. This is required to ensure compliance with approved source rules.
6) Submit a Time instead of Temperature for Food Safety procedure for the Ice bath holding process used during operation. See handout with inspection email.
7) Ensure all food employees have been trained on what symptoms and confirmed illness is required to be reported to managers and provide verification during inspections.
8) Ensure a Vomit and Diarrheal clean-up kit and procedure is available.
9) If the mobile smoker is to be used with the Mobile FSO license, the smoker trailer will need to be inspected and included on the equipment list and layout.
