[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system (floor drain) and a drain originating from equipment (ice machine) in which food are placed at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food are placed. Provide a proper air gap for the drainage of the ice machine to meet the requirements of this rule.

3717-1-02.2(F)(2) / Fingernails - artificial
Observed food employees with polished nails preparing beverages at the time of inspection.
To prevent contamination, unless wearing intact gloves in good repair, a food employee may not wear fingernail polish or artificial nails when working with exposed food. Employees with polished nails shall wear gloves or remove the nail polish.
3717-1-04.1(KK) / Food equipment - certification and classification.
Observed equipment (reach-in freezer) that is not approved by a recognized testing agency at the time of inspection.
Food equipment that is acceptable for use in a RFE shall be approved by a recognized food equipment testing agency. All equipment shall be commercial.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed single-service and single-use articles (cups and lids) that were not properly stored at the time of inspection.
To prevent contamination, single-service and single-use articles shall be kept in the original protective package or stored by using other means that afford protection from contamination until used. Provide protection for the cups and lids to meet the requirements of this rule.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Observed unnecessary or nonfunctional items in the back storage area at the time of inspection.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the RFE (retail food establishment) such as equipment that is nonfunctional or no longer used. Remove items that are no longer used for the RFE. Recommend reorganizing the back area to allow for more storage space.
