Critical Insufficient air gap between the flood rim and the water supply inlet for the walk-in condensation line.

Repeat Facility does not have an employee with manager certification in food protection.

3717-1-04.8(F) / Storage - prohibitions - utensils, equipment and linens.
Laundered linens stored in prohibited area. Observed wiping clothes stored on the air drying rack exposed and touching employee jackets.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted.Observed the asket for the third door of the make cooler to be damaged. Repair or replace as needed.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed accumulation of food rseidue and debris on the warming rack and shelving throughout the facility. Observed accumulation of debris on the pizza oven.

Observed floors under equipment not maintained clean.
