[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(C)(16) / PIC: duties - ensure written procedures and plans are maintained and implemented in the facility.
Critical Written procedures and plans are not maintained and implemented. A food defense or standard procedure for closing and opening is required due to the FSO not being able to closed to unauthorized access when not in operation and the Library is open to the public. Provide a detailed plan or a means to properly prevent unauthorized access to the FSO preparation areas when closed.

3717-1-02.2 (C ) / Hands and arms: when to wash
Critical Corrected During Inspection Food employee(s) did not wash hands when required. Observed employee drop food onto the floor, pick up and discard into waste container. Gloves were changed without proper hand washing. Manager discussed with employee regarding when tasks are broken, gloves are to be changed and hands properly washed before donning gloves.

3717-1-02.3(C) / Hair restraints - effectiveness.
Food employee(s) not wearing a hair restraint. Observe multiple food employees with exposed ponytails. Ponytails, braids, deadlocks, etc shall be proper covered.

3717-1-04.4(Q)(3) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Water temperature measuring device was not in good repair or accurate. Observed the warewashing final rinse temperature to register at 177 degrees F. Additional testing demonstrated the warewashing machine was properly sanitizing and the gauge may need adjusted.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed 3 compartment sin faucet to be corroded and slightly leaking. Repair or replace fixture.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed floors under and around the warewashing machine to be in need of cleaning. Warewashing machine shall be made easily movable to allow employees to easily clean around and under the equipment.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed multiple holes in FRP wall material around the 3 compartment sink left from the previous chemical dispensing equipment. Seal or repair holes.

Comments
1) Each shift is required to have at least one person with PIC certifications. At least one supervisor with ODH managers certification in Food Protection shall be assigned to this location (not required to be present at inspection).
2) Provide verification that all employees present have been properly trained on the symptoms that are required to be reported and confirmed illnesses.
3) If food handlers are delivering the foods to the hands of customers, gloves shall be considered contaminated. Gloves shall be change and hands are to washed before new gloves are donned. One person shall be handling the delivery of the foods to waiting customers.
4) Ensure the location has been provide with a proper vomit and diarrheal clean-up kit with a workable procedure that meets the ODH guidance.
5) A food defense plan is required to ensure the FSO is protected. Plan shall include details on how the staff will ensure the protection of the FSO when not in operation and the Library is open to the public. Plans for set-up and closure that cover the steps the staff with take to ensure the safety is needed.
6) Post original License at the location.
7) Post a ODH Coking poster at the location

A follow-up inspection will be completed 9/24 to review all violations documented in this report but mostly to review and approved the food defense plan.
