Critical Repeat TCS foods not being cold held at the proper temperature. Observed multiple cooked chicken items (cooling), cut cabbage, raw chicken, were not holding at acceptable cold temperatures.

Critical Repeat Refrigerated, ready-to-eat, TCS foods not properly date marked. Multiple foods (cooked meats, cooked fish, open tofu, etc) were not properly date marked as required. Date marking and labeling needs improvement.

Critical Repeat Presence of live insects. Observed live insects in monitoring traps under the dish machine and live pest were observed under the wok line (drip tray). Manager stated that Pest Control Operator has been scheduled for early next week. Cleaning needs to improve and unnecessary items need removed to reduce pest harborage.

Repeat Non-food contact surface(s) not cleaned at the required frequency. Observed accumulation of food debris and residues on doors to walk-in and reach-in cold holding equipment.

Repeat Facility not maintained clean. Observed floors in hard to reach areas, under equipment, under 3 compartment sink, under the fish holding tank, multiple locations throughout the FSO, to be in need of cleaning.

Repeat Physical facilities not maintained in good repair. Observed holes in dry wall in both restrooms. Observed damaged to hand sink cabinets. Restrooms are in need of repairs/updating.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. Observed an unused fish tank. Remove fish tank. Observed multiple equipment in bar and food prep that are no longer used. Remove all unnecessary items from the FSO.

Repeat Physical facilities not maintained in good repair. Observed flooring in the kitchen to be missing or has multiple damaged floor tiles. Observed floor an wall junctures were missing coved base molding. Repair or replace flooring.

Repeat Light intensity less than fifty foot candles in required areas. Observed measured light intensity to be less then 50 foot candles where employees work with knives and slicers at the time of inspection

Chlorine sanitizing solution at incorrect temperature and/or concentration. The warewashing machine was not provided with the proper sanitizer concentration. Discontinue use until testing kit demonstrates proper concentration.

Food equipment surfaces not cleaned at required frequency. Observed the raw chicken bowl was covered in food debris and residues at the time of inspection. Clean the bowl after each use.

Food employee(s) nails not maintained properly. Employees need to trim and clean fingernails.
3717-1-03.2(G)/Washing fruits and vegetables.
Raw fruit and/or vegetables not washed or washed improperly. Food Preparation sink was blocked and food employee was observed washing foods in the 3 compartment sink. Foods shall be only washed in a clean food preparation sink.

In-use utensils improperly stored. Observed rice scoops in dirty room temperature water.
3717-1-04.8(E)(1)/Single-service and single-use articles - storage
[52] Improper storage of single-service and single-use articles. Single use item storage needs to improve. Remove unnecessary items and organize.
3717-1-04.4(P)/Warewashing equipment - determining chemical sanitizer concentration.
Sanitizer test kit or other device not being used. Observed the sanitizer to be low or not available at the warewashing machine. Staff is not testing as required.
Comments
Re-inspection was postponed per the request of the owner. Owner was not available at the time of inspection and asked for inspection to be complete the following day 9/9.

Notes:
1) Replace lighting in the front section of the kitchen (dry storage area behind the warewashing machine station.
2) Remove the soiled carpet from the kitchen (front kitchen area).
3) Clean pre-rinse spray nozzle at the ware washing machine station.
4) Clean microwaves every 24 hours.
5) Properly discharge the oil water in the recycling bin behind the FSO. Container was used to soak hood vents/filters. This water needs to disposed in a sanitary sewer not the storm

Due to the amount of repeat and critical violations, a follow-up inspection is required. Due to the history of the FSO failing to comply with the Food Code, the FSO will need to continue to demonstrate significant efforts to improve to avoid additional enforcement action from the Licensor.

Pest issue needs to be addressed ASAP.
