| This mobile FSO truck has undergone a standard pre-licensing inspection (not in operation at the time of inspection) and has been approved to renew the MFSO with the understanding the following will be completed before operating: 1) Make the repairs to all cold holding unit that were not working at the time of inspection. Cold food options need to be limited until repairs are made. 2) Recommend servicing the potable water containers to ensure water is potable. ODA processor registration was provided at the time of inspection. Signed application with payment was received the day of inspection. All notes and violations will be re-checked at the next standard inspection. Okay to operate License will be mailed once processed. |
