Comments
This Food Service Operation has been approved to be issued a license with the understanding that the following will be completed:
1) Splash guards shall be installed right side of the cleaning equipment storage rack next to Grazers food preparation sink.
2) Additional lighting will be provided in the walk-in freezer (5 foot candles at the time of inspection, should be 10). Provide a new shatterproof light bulb in the prepackage display cooler (behind the counter) and left side of tall reach-in cold holding.
3) Repair hand washing sink to operational (work order was submitted at the time of inspection). Operation shall not operate until repaired.


A signed application and payment for the amount of $628 for the License #JSMH-APNPNZ. was received at the time of inspection. Okay to operate. License will be mailed to parent company/owner location. Original license must be provide at FSO location at all times.

Notes:
License has one restriction. 1) FSO must use the 3 compartment sink and food preparation sink in the adjacent FSO (Wow Bao). Walk-in freezer within the FSO will be shared with Grazers.

Diarrheal and Vomiting procdures shall be developed for this food service operation. Please see hand outs provided with inspection email.
Provide at least one person in with Level 1 PIC food safety training for each shift.
Provide at least one person with supervisory duties with a Level 2 Certification in Food Protection.
