Critical No approved backflow prevention device or documentation of interior device on the plumbing system connected to the Bubly Dispense equipment.

3717-1-03.2(G) / Washing fruits and vegetables.
Corrected During Inspection Raw fruit and/or vegetables not washed or washed improperly. Observed employee to be preparing cut leafy greens and spring greens without properly washing as the box states. Box label claims all lettuce shall be washed.

[52] Improper storage of single-service and single-use articles. Observed the sandwich wrapping paper to be exposed to customers has the move past the sneeze guard to the register. A short movable plastic sign holder has been place in this area and does not provided the needed sneeze guard protection as required. Provide a proper sneeze guard and/or protective container for the papers.

Single-service and single-use articles not protected from contamination. Observed single use beverage contact were removed from the protective plastic and the lip contact areas were exposed.

3717-1-04.4(P) / Warewashing equipment - determining chemical sanitizer concentration.
Sanitizer test kit or other device not being used. Due to the being not testing kit available, the staff are not monitoring sanitizer concentration. Monitoring the dish machine sanitizer solution shall be completed each shift or at least daily.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
No sanitizer test kit available was available for the low temp dish machine. Provide a chlorine test kit and train staff to test each shift.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed a hole in the floor of the walk-in cooler. The cap to the connector screw is missing. Replace cap or caulk hole to allow for easy cleaning of floors.

Comments
This inspection will be considered the 30 day inspection. Overall, the FSO clean and staff were knowledgeable.

Notes:
1) Adjust prep coolers to be able to hold all TCS foods at 41 degrees F or below through busy serving times.
2) Each shift is required to have at least one person with PIC certifications. At least one supervisor with ODH managers certification in Food Protection shall be assigned to this location (not required to be present at inspection).
3) If food handlers are delivering the foods to the hands of customers, gloves shall be considered contaminated. Gloves shall be change and hands are to washed before new gloves are donned. One person shall be handling the delivery of the foods to waiting customers.
4) Gallons of dairy, almond, and soy milk shall be proper date marked for a 7 day discard once opened.
5) Older deli slicing equipment shall be replaced after a certain period of time or when the equipment is found to be hard to clean and maintain. Ensure staff is properly washing, rinsing, and ware washing deli slicer at least every 4 hours, before and after each use.
6) Provide proper back flow on the BUBLY customer self dispensing equipment. (direct water line connect and over flow air gap).
