Comments
The PIC requested an inspection due to the recent facility review of the altered kitchen.
The facility alterations have been approved with the agreement from the PIC to address the following items:

1. Provide a mop sink. The PIC stated that the shower stall in the men's restroom and/or a floor drain in the maintenance room may be converted into a working mop sink.
2. Drop in risk class. The FSO can drop from a risk level two to a risk level one by signing the enclosed form. The license will be restricted to risk class one activities. All foods must be served in original packages (cheese, yogurt, fruit, crackers, etc.). The FSO can not prepare, wash, cut, or handle unpackaged foods (apples, oranges, etc.).
3. Non-commercial equipment. The FSO currently has two non-commercial cold holding units. When these need replaced, purchase commercial units.
4. Lighting. The kitchen does not meet lighting requirements of 20 footcandles. Brighter light bulbs and/or additional fixtures may resolve this issue.

Restrictions will be added to the FSO license due to the lack of sinks and risk level activities permitted.
Each of the outstanding items listed will be checked during the next standard inspection.
A 30-day inspection is not required for this existing facility.
