Repeat Non-food contact surfaces of equipment are unclean. Observed sidewalk from backdoor to the outdoor refuse storage area to be stained with food residues. Clean the sidewalk Refuse storage area.

3717-1-02.4(C)(11)/PIC: duties - ensure employees are proplery sanitizing cleaned equipment and utensils
PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters. Observed dish machine to be in-use and not functioning properly. All items in the FSO shall be washed rinsed and sanitized in the 3 compartment sink. All in use items are required to be washed, rinsed and sanitized every 4 hours. Ti limit the growth of harmful organisms. This included the spatulas and burger and fry scoops. The sandwich prep shall be washed rinsed and sanitized every 4 hours.

Equipment food-contact surfaces or utensils are unclean. Front service locations: 1) Fryer station, 2) hot holding try equipment, 3) food preparation sink, 4) burger spatula were in need of cleaning at the time of inspection.

Chlorine sanitizing solution at incorrect temperature and/or concentration on the dish machine. Have the machine proper serviced before use. Recommend developing a log to ensure staff is checking the solutions before use.

Food employees were not wearing a hair restraint/covering.
3717-1-03.2(M)/Wiping cloths - use limitation.
Improper use and/or maintenance of wiping cloths. Cloths were not provided in proper sanitizer solutions.
3717-1-04.8(E)(3)/Single-service articles and single-use articles - storage.
Single-service and single-use articles not protected from contamination. Observed multiple boxes of single use items stored on the floor of back room. Observed single use items to be stored directly under the Bug Light.
3717-1-04.4(P)/Warewashing equipment - determining chemical sanitizer concentration.
Sanitizer test kit or other device not being used. Ensure staff is using a test kit to keep solutions (in wipe buckets, dish machine, and 3 compartment sink) before use.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Facility not maintained clean. Observed outdoor refuse storage area to have accumulation of litter and debris. Concrete is soiled with food residues.
3717-1-04.5(A)(3)/Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed self-service area and soda dispensing equipment to be in need of cleaning. Recommend washing, rinsing, and sanitizing this area every 4 hours.
Comments
Observed the warewashing machine to operating properly at the time of inspection. Recommend using a log to monitor the sanitizer for the warewashing machine and the 3 compartment sink dispenser. Employees need to using the testing kit often to ensure proper warewashing is occurring each time.

Ensure all staff that was assigned to warewashing tasks understand how to use the equipment and chemicals. It is important to ensure that the proper flow of dirty and clean items is followed to prevent contamination to clean items.

PIC present were able to demonstrate knowledge with food safety. Observed many of the violations observed at the previous inspection have been correct. Continue to maintain compliance with the Ohio food rules to avoid additional enforcement action from the licensor.
