Comments
This inspection was completed in response to a request from the new owner. The following items shall be completed in order for a FSO risk class IV license to be issued.
1) Provide documentation that the grease interceptor was serviced and inspected by a licensed plumber. If repairs or replacement is recommended, consult with the building department.
2) Replace the door seal in 2 door reach-in cold holding equipment and base seal on walk-in cold holding facility. Check all cold holding and freezer seals and replace as needed.
3) Clean all door seals on all equipment.
4) Replace the shelves in cold holding equipment/freezer equipment that is rusty or paint is chipping.
5) Ensure all cold holding and hot holding equipment has been provided with a proper temperature measuring device.
6) All storage shelves shall have the bottom shelf installed 6 inches above the floor. Multiple shelves need elevated.
7) Caulk all floor and wall junctures on the walk-in cold holding unit. Provide a coved base finish. Coved base molding may also be installed.
8) Caulk and seal all gaps around the FRP inside and outside of the walk-in cold holding unit.
9) Repair or replace all FRP and corner molding inside and outside of the walk-in cold holding unit as needed.
10) Provide a replacement hand for the Single door Reach-in Freezer.
11) Single door reach-in freezer shall be elevated up 6 inches from the floor or made movable for easy cleaning.
12) Non-commercial chest freezer shall be replaced due to the damaged interior walls exposing the installation.
13) Caulk the wall junctures around the mop sink. Ensure a proper mop hanger has been provided.
14) Install a proper hand washing sink in the back preparation room. Recommend using the existing plumbing. Provide paper towels, soap and proper hand washing sign.
15) Ensure all light throughout the preparation area and storage areas are provided with shatter proof light or proper light shields.
16) Provide a signed application (to be provided at the final pre-licensing inspection) and license fee payment for the license #JSMH-B96REG.

Items that can be completed after a license is issued:
1) Clean and repaint the ceiling with a easily cleanable paint that is durable. If possible install a drop ceiling with kitchen grade ceiling tiles when able. Ceiling surfaces shall be smooth, durable and easily cleanable.
2) Recommend consulting with the Kent Fire Department on equipment set-up to ensure compliance with the Fire Safety Code.
3) Continue to work on removing unnecessary items from the facility such as unused equipment, non-commercial equipment, and old single use items that will not be used.
4) The following non-commercial equipment has been approved for use temporarily and shall be replaced with commercial grade (NSF or equivalent testing agency) equipment once the equipment is no longer in proper working order. a) Frigidaire Freezer - FFUZ1M7HWQ b) Kitchen Aid Mixer - KSM90WW c) Kitchen Aid Mixer - KP26M1XNP. These equipment items will be place on the back of the license as a limitation.
5) Clean floor and shelves in the catering van. Seal wooden shelves with a polyurethane or similar material or replaced with approved material (not wood). Painting is not recommend due to chipping.
6) Wooden shelving in the upstairs dry storage room shall be elevated up off the floor or sealed to the floor. Seal wooden shelves with a polyurethane or similar material or replaced with approved material (not wood).
7) Wooden Shelving over the food preparation sink shall be replaced with approved commercial grade shelving.

The following will be considered as limitations and included on the back of the license:
1) The following non-commercial equipment has been approved for use temporarily and shall be replaced with commercial grade (NSF or equivalent testing agency) equipment once the equipment is no longer in proper working order. a) Frigidaire Freezer - model # - FFUZ1M7HWQ b) Kitchen Aid Mixer KSM90WW c) Kitchen Aid Mixer KP26M1XNP.
2) The RFE Bent Tree coffee is connected to the FSO through a single door way. This doorway is to remain unlocked at all time so the coffee shop has access to the a shared mop sink.
3) FSO has not been supplied with an Ice Machine. All ice shall be supplied by an approved source and stored in the approved outdoor freezer. This freezer shall remained locked at all times.

As many items of the 1-16 shall be completed before a license will be issued. A final inspection has been scheduled for Friday March 1st at 1:00 PM.
