This inspection was conducted in response to a Change in Ownership from Best Fortune to Zhong Ming Inc. and a completed facility review.
The operator has made enough changes to the facility and the change in Ownership has been approved.
In order for this change to occur please completed the following:
Please review the renewal application and make any necessary changes.
Return the signed renewal application with a correct license fee amount of $528.00 for a Risk Level 4 FSO before March 1st 2015.

A New 3 door freezer has been added Make:Beverage Air, Model#: HBF72-1
A New Deli meat slicer has been added. Make: Globe, Model#: G12

The following issues were observed at the time of inspection and must be completed or repaired:
1) Replace the water filter on the Ice Machine - Make sure it is properly dated for next service day.
2) Remove the wood laminate counter tops from the kitchen area (service line and ware washing area). Wood is not a acceptable material to be used with a licensed FSO kitchen.
3) Repair the leaking water fixture on the 3 compartment ware washing sink.
4) Observed primary hot water generator was 75,000 BTU. Once this hot water generator is no longer in good working order, it must be replaced with a 199,999 BTU hot water generator. See plan approval letter and handout provided at the final pre-licensing inspection.
5) Repair all the missing or damaged floor tiles throughout the food preparation area.
