[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Corrected During Inspection Handwashing sink not accessible. Observed items being stored on the handwashing sink. PIC removed items from the sink and made the sink accessible.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed frozen chicken feet repackaged being stored above repackaged fish, beef, and pork packages in the glass freezers. PIC reorganized shelf to store based on cooking temperature of repackaged foods.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed four packages of pork packaged in the facility being left out at the front checkout counter. PIC voluntarily discarded product and stated the refrigerator is in process of being repaired.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Damaged flooring in front of the walk-ins. Walk-in lights were observed burned out and need of replacement.

Ensure items that state "keep refrigerated" are being held cold under 41 degrees F. Provide documentation if the product is for quality and not for food safety.
Continue to work on removing items that are no longer used in the facility or items that are not used for daily maintenance of the facility.

The violations documented are to be corrected and will be checked during next standard inspection.
