| This pre-licensing inspection was completed per the request from the owner for approval to be issued a Risk Level IV FSO license. The facility has been approved with the understanding the following will be provided or followed: Bar Area 1) An additional hand washing sink was being installed next to the three compartment sink at the bar. Manager stated they plan to redo the middle island of the bar and Tap area. This would limit the employees working on the front bar access to the hand washing sink located on the back bar (may be completed at a later time but must be installed before the 30 day inspection). Ensure soap, means to dry hands and hand washing signage has been provided at this new hand washing sink. 2) Provide a smooth, durable and easily cleanable finish on seal around the support beam and bar top juncture. Contractor was in the process of completing this at the time of inspection. Kitchen/Hallway/Dining room 1) Finish the customer side of the front bar (closest to Erie Street) where flooring and trim work is missing. Provide a coved base finish. 2) Provide 10 foot candles of light at times of cleaning in the dining/bar area (may be completed at a later time but must be installed before the 30 day inspection). General 1) Provide documentation that the Grease interceptor has been serviced and that a licensed plumber has inspected the unit to be properly functioning. 2) Backflow on main water supply shall be tested. Provide documentation that device is working correctly. Both are scheduled to be completed Thursday November 15th. Manager will email reports once completed. Observed a final Building/Fire department approval report to occupy the building. A signed application and license fee (check # 1020) payment for the amount of $630 for license JSMH-B58H9Q was received at the time of inspection. License will mailed to the mailing address provided on the application. Okay to operate. |
