[?] A summary of the violations found during the inspection are listed below.

3717-1-03.5(E) / Consumer advisory.
Critical The consumer is not properly advised of the risk of consuming raw or undercooked animal foods.
Except for whole-muscle intact beef steaks, if an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw, undercooked, or without otherwise being processed to eliminate pathogens, either in ready-to-eat form, or as a raw ingredient in another ready-to-eat food, the license holder shall inform consumers of the significantly increased risk of consuming such food by way of a disclosure and reminder as described in this rule.
3717-1-05.3(C) / Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment (Ice machine in kitchen, ice bin and ice bath in the server area) in which food, portable equipment, or utensils are placed.
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. Provide proper air gaps at these locations.

3717-1-04.1(KK) / Food equipment - certification and classification.
Observed deli slicing equipment is not approved by a recognized testing agency at the time of inspection. Deli slicing equipment is no longer approved by the national sanitization foundation.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. Replace deli slicer with a new and approved ( by NSF or similar testing agency) equipment.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed door seals on the griddle reach-in cooler to be damaged at the time of inspection. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair damaged door seals on all coolers with damaged door seals.
3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Observed re-use of single-service or single-use articles (scoop for the coleslaw, white containers, and dressing containers) at the time of inspection.
Single-service and single-use articles may not be reused. Replace all single use containers with containers that have been approved by NSF or a similar testing agency.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed the underside of the pass through window to be dirty. Observed the interior of the chicken cooler to be dirty. Observed multiple shelves in walk-in cooler to be dirty. Non-food contact surfaces of equipment are dirty at the time of inspection.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-04.5(C) / Cooking and baking equipment - cleaning frequency
Observed the interior of the convection oven and the interior of the microwaves not cleaned at the required frequency at the time of inspection.
Cooking and baking equipment and/or cavities and door seals of microwaves shall be cleaned at least every twenty-four hours. Clean ovens and microwaves as often as necessary to maintain equipment clean.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed accumulation of soil residue on nonfood-contact surfaces. Observed door seals on multiple reach-in coolers to have accumulation of food residues at the time of inspection.
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
3717-1-04.8(A) / Equipment and utensils - air-drying required.
Observed beverage containers were not being air dried before moved into storage area at the time of inspection..
After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food and utensils, and may not be cloth dried. Recommend installing a beverage container rack system that will allow for beverage containers to be properly stored and air dried.
3717-1-05.4(B) / Outdoor storage surface.
Outdoor refuse storage surface is not properly constructed, and sloped to drain.
An outdoor storage surface for refuse, recyclables, or returnables shall be constructed of nonabsorbent material and shall be smooth, durable, and sloped to drain.
3717-1-06.1(J) / Heating, ventilating, and air conditioning system vents.
Observed the ventilating hood systems was not properly designed and installed to prevent possible contamination to food contact surfaces at the time of inspection.
Heating, ventilating, and air conditioning systems shall be designed and installed according to the Ohio building code and so that make-up air intake and exhaust vents do not cause contamination of food, food-contact surfaces, equipment, or utensils. Re-install ventilation system to air is not longer blowing directly on the to food contact surfaces.
3717-1-06.4(A) / Repairing.
Observed the back screen door to the kitchen to be in poor condition. The physical facilities are not being maintained in good repair at the time of inspection.
Repair or replace screen door to help prevent pest enter into the FSO. The physical facilities shall be maintained in good repair.

All food employees shall demonstrate proper hair restraint at all times of operation.
The following must be completed to receive a final facility approval for the City of Kent Health department:
1) Replace a damaged or missing floor tiles and repair grout under the ware washing sink. Fill holes in floor from old electrical lines. Provide a smooth durable and easily cleanable floor.
2) Provide the dirty side of the ware washing stations to be self draining.
3) Seal all ware washing tables to wall.
4) Finish install all stainless steel, provide finish seals, provide a smooth durable and easily cleanable surface on all walls in kitchen area including to the right of the food preparations sink.
5) Provide a smooth durable and easily cleanable surface behind the food preparation sink (FRP or similar material)
6) Condensation drainage from the walk-in cooler and freezer shall be draining to a drain to a public sewerage line
7) Drainage lines from the soda dispenser machine shall be installed off the floor at least 6 inches above the floor. Drainage from the ice bin needs to properly air gapped. All utility lines shall be install off the floor for easier cleaning.
8) Provide a copy final approval from the building and fire department inspection
