Comments
This inspection was completed per the request of the operator to approve the mobile food trailer to be issued a MFSO license. A signed applications and License fee for the amount of $330(Check# 9913) was received at the time of inspection. The unit was not in operation at the time of inspection. Observed cold holding equipment to be functioning correctly at the time of inspection. Hand washing sinks and 3 compartment sinks were observed to be operational and in proper working order. Inspection was completed with Fire Prevention inspectors.

Okay to Operate.

Notes:
1) Caulk all seems in the floor and install coved base trim.
2) Seal wood counter top. Wood will need to covered with a durable cover if not maintained.
3) Seal all holes, seems, and junctures around the counter tops/screw holes.
4) Provide a proper sign with characters 3 inch by inch in size.
5) Ensure all food employees have been trained on what symptoms and confirmed illness is required to be reported to managers and provide verification during inspections.
6) Ensure a Vomit and Diarrheal clean-up kit and procedure is available.
7) An air gap is required between the grey water tank and drain pipe to prevent back-up and contamination to the 3 compartment sink.
8) Provide a proper sanitizer test kit and metal stem temperature measuring device.
9) PIC certification needs to be present when in operation.
