[?] A summary of the violations found during the inspection are listed below.

3717-1-04.1(KK) / Food equipment - certification and classification.
Observed equipment (household refrigerator, chest freezer) that was not approved by a recognized testing agency at the time of inspection.
To prevent health hazards, food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. Approved commercial equipment shall be used.
3717-1-04.4(A)(3) / Equipment - cutting or piercing parts of can opener not maintained.
Observed the can opener blade to be dirty at the time of inspection.
To prevent contamination, cutting or piercing parts of can openers shall be kept clean. The can opener shall be cleaned and maintained.
3717-1-04.4(Q) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Observed the final rinse temperature gauge on the warewashing machine that was not kept in good repair or calibration at the time of inspection.
To ensure equipment is functioning properly, water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use. The final rinse temperature dial shall be maintained in good repair.
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
The plumbing fixture for the warewashing sink behind the bar was not properly maintained (leaking) at the time of inspection.
To prevent health hazards, a plumbing fixture shall be properly maintained. Repair the plumbing fixture for the warewashing sink.

Comments
Overall the facility was observed to be in good order.
Recommend minimizing the amount of times the reach-in cold holding unit is opened and closed.
A level 2 ODH certification in food safety training is required to be obtained for one operator of the facility by March 1, 2017.
A new walk-in refrigeration unit has been installed without approval from the Health Department. A facility review is required and the FSO will need to submit a plan review.
