| This inspection was conducted per the request from the manager/operator. In order for the KCHD to approve the facility to be issued a FSO license, the following shall be corrected or provided: Bar 1) Provide an additional hand washing sink next to the three compartment sink at the bar. Manager stated they plan to redo the middle island of the bar and Tap area. This would limit the employees working on the front bar access to the hand washing sink located on the back bar (may be completed at a later time but must be installed before the 30 day inspection). 2) Provide a smooth, durable and easily cleanable finish on seal around the support beam and bar top juncture. Kitchen/Hallway/Dining room 1) Finish the corner/shelf of the new half wall above server hand washing sink. Seal the under side of the shelf, caulk in the gap under the the shelf where wood meets the wall. 2) Caulk and seal corners to the pass through window. 3) Caulk and seal all floor and wall junctures that are not provided with a toe molding or cover base molding. 4) Provide 10 foot candles of light at times of cleaning in the dining/bar area (may be completed at a later time but must be installed before the 30 day inspection). General 1) Provide a final Building/Fire department approval. 2) Provide a signed application (to be provide at final pre-licensing inspection) and license fee payment for the amount of $630 (accepted at final). 3) Provide documentation that the Grease interceptor has been serviced and that a licensed plumber has inspected the unit to be properly functioning. 4) Backflow on main water supply shall be tested. Provide documentation that device is working correctly. This is not a final list and subject to change upon additional review of the facility. Next Pre-licensing inspection has been tentatively scheduled for Wednesday November 14th at 1:00 PM. |
