Comments
This inspection was completed per the request of the new owner to complete the facility review process. The FSO has been approved to be issued a Risk Level II FSO license with the understanding the following will be completed:
1) Submit a signed license application with license Fee $285.
2) Provide a signed action plan letter indicating when larger issues will be corrected by the owner. Please include a deadline for each item that additional time is needed.
3) Provide proper backflow prevention devices on the Ice maker and on the Multi-flow system.
4) Remove cross connection on the ice machine drainage lines. The ice maker drain shall not be contacted to the ice bin drainage. Two drains shall be separately draining to an air gapped drain.

The following items shall be corrected within 30 days or provided on the action plan letter:
1) Provide additional lighting in the back room. 20 foot candles of lighting is required at the 3 compartment sink. Additional lighting is need at each hand washing sink in reach rest room (20 foot candles).
2) Install a hand washing sink if the 3 compartment sink will be used for ware washing.
3) Install a Food Preparation sink. The 3 compartment sink may be converted to a food preparation sink by providing a proper air gap. This sinks hall be used for washing lemons and limes.
4) Provide shatter proof lighting over the Bar 3 compartment sink, within each reach in beer cooler.
5) Caulk and seal around each sink in the Bar and rest rooms.
6) Seal gap with caulk between the bar 3 compartment sink and ice bin
7) Provide a proper coved based finish around the base of the draft cabinet. All floor and wall junctures shall be coved and sealed.
8) Provide proper ventilation in each restroom.
9) Provide a signed letter indicating that when the existing hot water generator is in need of replacement that new generator will be 109,339 BTU (Gas), 22 Kw/hr (electric) or 9.0 GPM (tankless). Connect the KCHD when a replacement is needed. New install shall be permitted through the Kent Building Dept.

All items listed on this inspection will be re-checked at the 30 day inspection (to occur within 30 days). If items are not completed or included on the Action Plan letter, they will be violated according to the Ohio Food Code.

Okay to operate once the signed application and license fee is submitted.
