[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed the facility was not provided with a designated, properly installed sink at the time of inspection for preparing ready to eat foods.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. Provide a food preparation sink that is properly installed with an indirectly connected drainage to prevent contamination to foods. Until this sink is provided, all foods that need to be washed shall be washed at the Stanton Middle school kitchen.

3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed single-service and single-use articles were not properly stored at the time of inspection. Single-use articles were observed underneath the shelving unit next to the oven.
To prevent contamination, single-service articles, and single-use articles shall be properly stored: (a) In a clean, dry location; (b) Where they are not exposed to splash, dust, or other contamination; and (c) At least six inches (fifteen centimeters) above the floor.

Observed employees washing hands upon arrival to work and when required.
Drains in receptacles and waste handling units for refuse, recyclables, or returnables shall have drain plugs in place.
Women's restrooms shall be provided with a covered receptacle.
The facility's current employee health policy has 5 of the 13 illnesses that the Ohio Food Code requires to be reported. Refer to the rule 3717-1-02 (A)(3) for the complete list of illnesses that shall be included.
As discussed a handout will be attached to this report regarding the packaged foods that are monitored by employees.
