Comments
This FSO has been approved to be issued a Risk Classification III seasonal FSO License with the understanding that the following will be provided:
1) Please review the Sanitarian guidelines for licensing outdoor grilling and ensure the operation will be able to comply with the guidelines. No procedure will be required at this time.
2) Caulk all floor and wall junctures to create a Coved base finish.
3) Caulk around the plumbing and wall juncture/void under the 3 compartment sink.
4) Replace the Non-commercial microwave and approved (NSF or equivalent) microwave. PIC stated that is will be corrected before the 30 day inspection.
5) Proper hot water shall be provided at each sink. The Hand washing sinks shall be provided with at least 100 degrees F and the ware washing sink (3 compartment) shall be provided with at least 120 -140 Degrees F. A larger hot water tank may be needed or a separate hot water tank for only the FSO supply or even instant hot water equipment installed for the Hand washing sink and 3 compartment sink.
6) In order for the food to be processed and prepared at the Main Pizza Shop Angelo's Pizzeria in Rootstown, OH, RFE License # 381. Owners are currently in the process of applying and receiving approval from ODA for the Commercial Processor License. Provide with licensing/permit information once received from ODH to the Kent Health Department.
7) Remove paper towel dispenser from the 3 compartment ware washing area.
8) Provide the make and model numbers for the additional equipment planned to be brought to the concession stand (slushy machine, outdoor grill, fryers, etc.)
Additional equipment has been added since the last inspection ( hot holding - Carter Hoffman - PH1810, Worksman Cycles - 6062576, Victory Freezer - VF-1)
9) A signed application (provided at the next pre-licensing inspection) and payment for the amount of $508.00 made out to the Kent Health Department before operating.
10) Provide a Certificate of Occupancy from the Building/fire department or final building permit approvals. A copy is needed for final Health department approval.

If the above items are not provided or completed by the 30 day inspections, the items will be documented accordingly as violations.
ODH Level 2 certification must be provided by the 30 day inspection.
