[?] A summary of the violations found during the inspection are listed below.

3717-1-06.1(M) / Outer openings - protected.
Outer back door without proper protection against the entry of insects and rodents (not self-closing).

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Employee beverage were not being stored in designated locations away from food preparation. Ensure employee belongings are stored away from operation's products. Emplyoee drink did not have a lid and was stored next to the fryers.

3717-1-02.2(G) / Jewelry - prohibition.
Food employees wearing jewelry on arms or hands during food preparation.

3717-1-03.2(G) / Washing fruits and vegetables.
Observed banana sliced through the peel without washing or washed improperly next to the hand sink in the back prep area. See license limitation, the facility is not equipped with a properly plumber sink. Banana shall be peeled before slicing.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed ice build-up in the freezer. Products shall not be stored under leaking condensor lines.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of food residue and other debris on the following nonfood-contact surfaces of equipment: 1) Shelving in walk-in refrigeration. 2) Walls under the condensor and behind the shelving unit in the walk-in refigeration unit. 3) Gaskets and handles for multiple freezer and refrigeration units (damaged gaskets shall be repaired).

Walls and ceilings not smooth and easily cleanable throughout the facility. Observed unfinished surfaces and missing ceiling tiles within the facility.

Comments
Sanitizer and detergent at the three-compartment was almost empty and air was observed in lines. PIC changed out the detergent and sanitizer at the three-compartment sink.
Ensure the sanitizer concentration is being monitor to ensure proper sanitizing (200-400 ppm).
Handwashing sinks shall only be used for handwashing. Ensure hand sinks are accessible at all times.
Observed black tape across the ceiling in the back storage area with staining and evidence of water leak in the corner.
The walk-in light intensity shall be 10 foot candles during cleaning. Additional lighting may be needed.

The violations documented shall be corrected and will be checked during next standard inspection.
