Repeat Equipment and/or components not maintained. The door seal to the walk in cold holding unit is damaged.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Repeat Nonfood-contact surfaces constructed of unapproved materials. An unapproved absorbent mat was being used in the deli display cooler.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Repeat Nonfood-contact surfaces of the wooden storage shelves in the dry storage room are not easily cleanable.

Repeat Equipment and/or components not maintained. The paper towel dispenser in the deli area does not work properly.

Repeat Non-food contact surfaces of equipment are unclean. Multiple cold holding units are not clean (deli cold holding unit and the grates of the open display cold holding units).

Repeat No covered trash receptacle in women's restroom.

Repeat Facility not maintained clean. The walk in cold holding unit is not clean (shelves, floor, and fan).

Repeat Facility not maintained clean. Cabinet spaces are not clean in the deli area and coffee station (cupboards, drawers, and counters).

Repeat Physical facilities not maintained in good repair. The floors, walls, ceilings, and floor junctures are in disrepair in and around the bathroom.

Repeat Floors of the facility not maintained clean (dry storage room, freezer, walk in cold holding unit, bathroom, and under 3-compartment sink).

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items on premises. There is a large amount of old equipment on the premises (sandwich conveyer in the deli, cold holding unit in the deli, coolers in the dry storage room, and unnecessary items behind the building).

Repeat Physical facilities not maintained in good repair. Ice build-up on the walk in freezer condenser.

Repeat Physical facilities not maintained in good repair. Light bulbs were not working in the dry storage room, walk in cold holding unit, and above the bread shelf in the retail space.

Repeat Physical facilities not maintained in good repair. There are approximately 11 water damaged ceiling tiles in the retail space. The PIC states that the roof has leaked recently.

Repeat Light intensity less than ten foot candles in the walk in cold holding unit (5 foot candles).

The following violation(s) have been corrected since the last inspection.
3717-1-02.4(C)(13)/PIC: duties - ensure employees are properly trained in food safety, including food allergy awareness.
Person in charge did not ensure their employees are properly trained in food safety. The PIC was unable to demonstrate proper allergen information for the packaged and labeled products from the deli. Education was provided to the PIC about allergens. The PIC removed the deli salads from the open display unit until a permanent solution could be made for the labels.
3717-1-02.4(B)(13)/PIC: Demonstration of Knowledge - Identifcation and proper use of poisonous or toxic materials
PIC unable to demonstrate knowledge of toxic materials used and stored in the dry storage room. Education was provided to the PIC to make sure chemicals are properly stored and used going forward.
3717-1-02.4(B)(11)/PIC: Demonstration of Knowledge - Explaining correct procedures for cleaning and sanitizing
PIC unable to demonstrate knowledge of cleaning and sanitizing frequency of the tongs used for beef jerky. Education was provided to the PIC to make sure the tongs are properly ware washed at least every 24 hours going forward. The PIC placed the tongs in the 3-compartment sink to be ware washed.
3717-1-02.4(B)(12)/PIC: Demonstration of Knowledge - Identification and protection of water source
PIC unable to demonstrate knowledge of proper water source and preventing contamination regarding the proper backflow for the mop sink. The PIC removed the hose from the mop sink to eliminate the potential backflow risk.
3717-1-05.1(O)(1)/Using a handwashing sink - accessible at all times
Handwashing sink in the bathroom is not maintained to be accessible for employee use (appears to be frozen plumbing). The PIC called the plumber at time of inspection to request a repair service.
3717-1-03.5(C)(2)/Food labels - packaged food
Label on a food packaged in-house did not contain all required information. Items packaged in the deli (deli salads, peanuts, etc.) do not list the sub-ingredients.
3717-1-06.3 (C)(1)/Employee accommodations - designated areas
The employee personal lunches were improperly stored in the deli display cooler.
3717-1-06.4(F)/Drying mops.
Mop dried improperly in the mop bucket.
3717-1-06.4(A)/Repairing.
Physical facilities not maintained in good repair. There is a plumbing leak in the dry storage room near the bathroom.

All critical violations have been corrected. Continue to make progress correcting the remaining non-critical violations before the next standard inspection.
