Critical Repeat Person in charge did not ensure their employees are properly trained in food safety. PIC was unable to retain the labels for the dessert items in the dessert display case. PIC is able to use there personal phone to look up items. Provide labels for the dessert products within the facility.

Critical Employees are not informed in a verifiable manner of their responsibility to report information about their health.PIC was unable to find documentation at the facility regarding the operation's sick policy. Employees state they rememeber signing a policy.

3717-1-03 / Food - safe, unadulterated, and honestly presented
Food was unsafe, adulterated, or not honestly presented. Observed unthawed waffles in the dessert case that state to "keep frozen". Provide documentation this product is safe to leave out at room temperature or discontinue the activity.

3717-1-03.5(C)(1) / Food labels - packaged food
A food packaged in the facility not labeled. Packaged cookies did not contain a label with proper information (address, quantity, ingredients, etc.).

3717-1-04.2(C) / Drainboards.
Repeat Insufficient storage space provided for soiled and clean items.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Flooring not maintained clean in the warewashing room specifically around equipment and hard to reach areas.
