3717-1-02.4(B) / Person in charge: demonstration of knowledge.
Critical Observed the person in charge was unable to retain the label for the graham base product located next to the blizzard mixing equipment at the time of inspection.
To prevent or correct factors that may cause foodborne illness, based on the risks inherent to the food operation, during inspections and upon request, the person in charge shall retain labels for foods. Provide a label for the product listed above.
3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed a directly connected drain on the 3-compartment sink (used for food preparation) at time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine between its waste outlet and a floor drain when the machine is located within five feet (1.5 meters) of a trapped floor drain and the machine outlet is connected to the inlet side of a properly vented floor drain trap; or a warewashing and culinary sink. Provide a properly designed food prep sink.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items in the walk-in freezer at the time of inspection.
To prevent contamination, food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. Discontinue storing boxes directly on the floor.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed non-food contact surfaces of equipment dirty at the time of inspection. The interior of freezer 1 was found with food debris and the exterior of freezer 2 contains a large rust streak down the middle.
To prevent health hazards, nonfood-contact surfaces of equipment shall be kept clean. Clean and maintain the equipment surfaces listed above.
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
Repeat Observed the plumbing system on the water filters was not properly maintained near the back warewashing room at the time of inspection. A fitting for the plumbing system was leaking water onto the floor.
To prevent health hazards, a plumbing system shall be properly maintained. Repair the plumbing system listed above.
3717-1-05.3(I) / Other liquid wastes and rainwater.
Repeat Observed improper draining of condensation and other nonsewage liquids for the walk-in cold holding unit and for the ice bin next to the salad prep cold holding unit at the time of inspection.
To prevent health hazards, condensate drainage and other nonsewage liquids and rainwater shall be drained from point of discharge to disposal according to law. Provide proper drainage for the walk-in unit and ice bin.
3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Observed locations where the walls were not smooth and easily cleanable at the time of inspection. The FRP panels were off the walls and drywall was exposed.
To prevent health hazards, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Repair/replace walls to meet the requirements of this rule.
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Repeat Observed multiple floor and wall junctures throughout the facility that were not properly coved or closed at the time of inspection.
To prevent health hazards, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed. Floor and wall junctures shall be properly coved.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris under the shelving in the walk-in freezer and on the floors under freezer #1 and #2 at the time of inspection.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean and maintain the floors listed above. Bottom shelves shall be maintained up off the floor (atleast 6 inches).
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unnecessary or nonfunctional items in the back storage room at the time of inspection.
To prevent pest harborage, the premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO such as equipment that is nonfunctional or no longer used. Remove unnecessary items from the facility.

Condenser fan covers shall be cleaned as often as needed to remain clean.
