3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment (warewashing sink used to wash green peppers and cantaloupes) at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment (warewashing sink) in which food, portable equipment, or utensils are placed. Install a culinary sink that has a proper air gap. Recommend contacting a licensed plumber and please contact the building department. Fruits and vegetables shall be received pre-washed and cut if a sink is not installed.

3717-1-02.2(G) / Jewelry - prohibition.
Observed food employees wearing jewelry on arms or hands during food preparation and serving at the time of inspection.
To prevent contamination, except as provided by (G)(2) and (G)(3) of this rule, food employees may not wear jewelry on their arms or hands. Discontinue wearing jewelry while working with food.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. Observed stained ceiling tiles above the reach-in cold holding equipment at the time of inspection.
To prevent health hazards, the physical facilities shall be maintained in good repair. Investigate the cause of stained tiles and repair or replace tiles.

Observed the facility to be very organized and clean at the time of inspection.
Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized
The violations documented shall be corrected and will be checked during next standard inspection.
