3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods not properly date marked. Foods in the salad cold holding unit are not properly date marked (chicken, ham, tomatoes, and lettuce). Education was provided to the staff to improve date marking procedures.

Repeat Facility does not have an employee with manager certification in food protection.

There are multiple light bulbs not working in the kitchen. There are multiple damaged floor tiles in the kitchen.

Exhaust hoods are not clean and the floors in the hard to reach areas are not clean.
