3717-1-04.1(BB) / Warewashing machines - temperature measuring devices.
Critical Observed the dial for the final rinse temperature on the high temperature warewashing machine that was not functioning properly at the time of inspection.
To ensure proper sanitization, a warewashing machine shall be equipped with a working temperature measuring device that indicates the temperature of the water in each wash and rinse tank and as the water enters the hot water sanitizing final rinse manifold, or in the chemical sanitizing solution tank. The final rinse temperature dial shall be in good repair.
3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from the food preparation sink in which food, portable equipment, or utensils are placed at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine between its waste outlet and a floor drain when the machine is located within five feet (1.5 meters) of a trapped floor drain and the machine outlet is connected to the inlet side of a properly vented floor drain trap; or a warewashing and culinary sink. Provide an air gap for the preparation sink to meet the requirements of this rule.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed equipment (small household refrigerator) at the time of inspection that was not approved by a recognized testing agency.
To prevent health hazards, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Remove unapproved equipment.
3717-1-04.1(Y) / Temperature measuring devices.
Observed the fryer preparation cold holding unit that did not contain a temperature measuring device at the time of inspection.
To ensure equipment is functioning properly, a properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit. Provide a temperature measuring device for this unit to meet the requirements of this rule.
3717-1-06.2(I)(1) / Lighting - intensity (10)
Observed the measured light intensity in the basement walk-in cold holding unit (0 fc) was less than ten foot candles at the of inspection.
To encourage cleaning and for safety, the light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units. Provide proper lighting to meet the requirements of this rule.
