[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(B)(2)(i) / PIC: Demonstration of Knowledge - Identifying major food allergens
Critical PIC unable to retain labels for the frozen foods in the reach-in refrigeration unit and the freezer of the household refrigeration/freezer unit in the dry storage room.

3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Repeat Handwashing sink is being used for purposes other than handwashing. Observed plastic tub in the left side of the handwashing sink. Trays used for the new convection oven shall not be warewashed in this sink. A three compartment sink is needed to clean and sanitize equipment. Only disposable equipment and utensils shall be used. PIC stated the owner has been inquiring and receiving quotes about installing a three-compartment sink.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature in the infant room. PIC adjusted the refirgeration unit to hold foods under 41 degrees F.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Working food containers not properly labeled. Observed unmarked spray bottle on the top shelf of the storage rack in the storage room near the back door.

3717-1-03.2(G) / Washing fruits and vegetables.
Raw fruit and/or vegetables not washed or washed improperly. Observed unwashed blueberries and strawberries in the reach-in refrigeration unit and no proper food prep sink available for washing.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors in the dry storage room and in the main kitchen area not maintained clean. Items on the floor shall be moved to allow for easy cleaning and ensure floors are being maintained under and around equipment.

Ensure employees are informed in a verifiable manner of their responsibility to report information about their health (sick policy).
Flooring that is no longer smooth and easily cleanable shall be replaced to allow for easy cleaning. Flooring shall be free of cracks and crevices.
Ensure chemicals are stored in a designated location away from food products.
Operation shall continue to operate with the restriction of not having a three-compartment sink but will need to update the plan on when a sink will be installed.
Frozen items that are stated on the bag "keep frozen" shall be kept frozen until ready to use. Ensure these products are being properly date marked.

The violations documented shall be corrected and will be checked during next standard inspection.
