3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Corrected During Inspection Handwashing sink is being used for purposes other than handwashing. Observed handwashing sink being used as a dump sink. PIC stated they will discontinue using sink except for handwashing.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection.

3717-1-04.2(G)(1) / Food temperature measuring devices - availability
Food thermometer not readily accessible.

3717-1-06.3(D) / Distressed merchandise - segregation and location.
Damaged, spoiled, and/or recalled products not segregated from food, equipment, utensils, linens, and single-service and single-use articles.PIC discarded expired frozen pizzas from the chest freezer. Pizzas are not sold and were for the pizza operation that has not been open for an extended period of time.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed multiple peices of equipment throughout the facility no longer in good repair.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed exterior of coolers (doors, handles, gaskets, etc.) not maintained clean.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Toilet damaged, PIC states the toilet still works.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. Observed paint supplies and equipment and other items that are not used for the operation of the facility. Observed the back room full of old equipment and no longer in use.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed multiple damaged and stained ceiling tiles throughout the facility.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors throughout the facility not maintained clean.

3717-1-06.2(I)(2) / Lighting - intensity (20 FC)
Light intensity less than ten foot candles in required areas. Obsered measured light intensity in the back of the store at 0 foot candles. Observed multiple light bulbs that need changed out and are no longer working. Observed shop light temporarily installed to provide lighting. Observed light intensity measured at 0 FC at the hand sink in the restroom.

Comments
Ensure new hires are signing the FSO's employee health reporting agreement (sick policy).
The operation currently is not selling any TCS foods.
Per discussion with PIC, they plan to sell TCS foods at some point and will ensure cold holding units are holding foods below 41 degrees F.

Contact the Health department for an inspection prior to selling any hot foods.

The items listed below and on this report shall be completed prior to selling hot foods:
1. Approval from fire department.
2. Back room needs to be cleared out to have access to 3-compartment sink and mop sink.
3. Install hand sink in the back warewashing room.
4. Unnecessary items including broken equipment shall be removed or repaired (slicer, ice machine, etc.).
5. Chemicals stored in the open glass container (tobacco products, pipes, etc.) in the pizza prep area shall be relocated away from operation.
6. Storage for clean and dirty utensils and equipment shall be provided at the three-compartment sink.
7. Lighting shall be updated or repaired to comply with light intensity requirements (20 FC in back room, possibly 50 if food prep is going to occur)

The violations documented shall be corrected and will be checked during next standard inspection.
