Comments
This Food Service Operation (FSO) has undergone a final Facility Review inspection and has been approved. The facility is approved. Okay to Operate.

Items shall be submitted to the Kent Health Department:
A letter indicating that the Primary hot water Generator will be replaced with the properly sized unit when need (6 Kilowatts).
A letter requesting a drop in Risk Classification II. Provide details on why the drop is being requested. Indicate that no Risk Class III activities will occur.
Items that will be re-checked at the next Standard Inspection and documented as violations if not completed:
Caulk along the wall junctures of all the wall mounted sinks (3 compartment and hand washing sinks)
Install a working sink faucet fixture to allow for easy fill up of the 3 compartment
Storage shelves shall be installed off the floor 6 inches for easy cleaning.
Ice machine and bin drainage lines shall be install up off the floor to ensure floors are easily cleanable.
Provide shatter proof lighting over all locations where foods are prepared and utensils are stored/cleaned/etc.
The light intensity shall be at least twenty foot candles (two hundred fifteen lux): at a surface where food is provided for customer self-service such as buffets or salad bars or where fresh produce or packaged foods are sold or offered for human consumption; inside equipment such as reach-in and under-counter refrigerators and at a distance of thirty inches (seventy-five centimeters) above the floor in toilet rooms and in areas used for hand washing, warewashing, equipment storage, or utensil storage.
Provide documentation that a Licensed Pest controller has been provide service to the FSO. Documentation shall be obtained and will be reviewed at all standard inspections.
