| This inspection was conducted in response to a request from the owners for a final pre-licensing inspection. The facility was approved to be issued a license at the time of inspection. The owners moved from a RFE location at 425 Franklin Ave. This location was licensed as a risk level 4. A new license will be issued for new 123 N. Water St. location and the license fee from the 425 Franklin Ave. location will be transferred to this location. A 30 Day inspection will be required for the location. A signed application was received at the time of inspection. Limitation to be add on the back of the license: 1) The FSO will be sharing the refuse storage a McKay Bricker Framing 141 East Main Street and the recyclables at 152 East Main Street. These location will be inspected at each standard inspection. 2) Current primary hot water generator is undersized, owners have agreed to provided a replacement generator when existing is not longer in proper work order. See letter in Note to File. The following items shall be provide: 1) Temperature measuring devices shall be provided with in all cold/hot holding equipment. 2) Provide proper backflow prevention on Combine Oven and Convection Oven direct water connections. Provide the proper stand. 3) Provide hot water tank letter regarding the replacement when needed. 4) The New license will not be issued until the old RFE license is submitted to the Kent City Health Department. 5) Repair the 3 door cold holding equipment to hold at proper cold holding temperatures. Please contact the Kent Health Department when 1-5 have been completed. Items will be documented accordingly during the 30 day inspection. Okay to operate. |
