| This inspection was conducted in response to a Facility Review application for a new FSO license. In order for a FSO license to be issued, the following must be completed: 1) Install "Employee Must Wash Hands" signs, provide soap and paper towels (in dispenser) at all designated hand washing sinks including the restroom. 2) Provide a trash can with a lid in the women's rest room. 3) Provide an additional drying cart or shelf in the ware washing area. 4) Please review the Sanitarian guidelines for licensing outdoor grilling and ensure the operation will be able to comply with the guidelines. No procedure will be required at this time. 5) Caulk all floor and wall junctures to create a Coved base finish. 6) Caulk along the splash guards and wall junctures on all sinks (hand sinks, 3 compartment sinks, etc. ) Caulk along the back splash on the mop sink and the base of the mop sink to floor. 7) Seal or cover hole in floor in mop room/utility room. 9) Provide a Mop hanger. 10) Caulk around the plumbing and wall junctures/voids. Caulk along the service window and door frame. 11) Provide operators equipment list (Make and Model Numbers). Provide a layout of all the operation. 12) A signed application (provided at the next pre-licensing inspection) and payment for the amount of $508.00 made out to the Kent Health Department. 13) Provide a Certificate of Occupancy from the Building/fire department or final building permit approvals. A copy is needed for final Health department approval. Recommend looking over the Facility Review Approval Letter and provide all items applicable to the operations. ODH Level 2 certification must be provided by the 30 day inspection. See attached handouts. This is not a final list and is contingent upon additional inspections. |
