| This consultation inspection was completed as part of the pre-licensing process and per the request of the Manager/owner. The following list shall be completed before a FSO risk Level IV license will be issued for the facility: 1) Caulk the drain pipe and wall juncture under the hand washing sink. 2) Provide a paved or concrete surface under the refuse storage containers/dumpsters. Surface shall be graded to drain. 3) All light over the bar shall be shatter proof. Provide shatter proof bulbs without the glass globe or shatterproof globes or fixtures. 4) Provide at least 100 degrees F water at the women's restroom. 5) Remove the cross connection on the beer tap over flow and beer glass rinser drain. The there shall be an air gap before the drains are connected to another drainage line (condensation drainage). 6) Caulk around the kitchen 3 compartment sink grease interceptor and floor juncture. Provide a coved finish. 7) Seal all steel back splash junctures behind the hot line. 8) Dish machine water temperature gauge needs repaired or replaced 9) Provide a "bleed" drainage line on the backflow prevention device on soda dispensing compressor. 10) Finish installing Ice machine. Ensure drain from maker and bin are separated and provided with proper air gaps. Ensure a backflow prevention device is installed on the water connection. 11) Beer cooler and beer line cooler shall be draining to a sanitary sewer drain and provided with a proper air gap. 12) Provide a final building and fire approval and Certificate of Occupancy. 13) A signed application (application to be provided once final approve is provided) and proper license fee is require before operation.
