3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Observed package of raw beef stored and dripping on raw eggs in the beer walk-in cold holding unit at the time of inspection.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule. Raw animal foods shall be stored on shelves based on minimum cooking temperature (highest temperature at bottom, lowest temperature at top).
3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Observed refrigerated, ready-to-eat, TCS foods (hot dogs, lunch meat, ground beef, diced and sliced tomatoes, cooked pasta, multiple lettuce containers, sausage gravy) held refrigerated for more than 24 hours were not properly date marked at the time of inspection.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one. Date marking must improve. To ensure food safety, unmarked foods shall be discarded to ensure they have not passed the 7 day use/discard by.
3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Observed ready-to-eat, TCS food (opened coleslaw in walk-in) that had been date marked was not properly discarded when required at the time of inspection.
To prevent illness, refrigerated, ready-to-eat TCS food that has been date marked in the facility shall be discarded if it: (a) exceeds the temperature or time specified in (G)(1) of this rule, except the time the food is frozen; (b) is in a container or package that does not bear a date or day; or (c) is marked with a date or day that exceeds the time and temperature combination specified in (G)(1) of this rule. Out dated (past 7 day) TCS foods shall be discarded. Ensure employees are monitoring dates.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed an accumulation of debris in the ice scoop holder and the interior of the ice machine at the time of inspection.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Clean and maintain the ice scoop holder and ice machine.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed equipment (freezer, fryers, blender, keurig, microwave, containers holding food, etc.) not approved by a recognized testing agency at the time of inspection.
To prevent health hazards, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Approved commercial equipment shall be used in the FSO.
3717-1-04.2(H)(2) / Temperature measuring devices - mechanical warewashing.
Repeat Observed a missing irreversible registering temperature indicator (thermometer or labels) for the hot water mechanical warewashing machine at the time of inspection.
To ensure proper sanitizing, a temperature measuring device is essential to monitor mechanical ware washing to ensure sanitization. Provide stickers or a maximum reading thermometer.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed torn gaskets on the ice machine and front reach-in cold holding unit at the time of inspection.
TO prevent health hazards, equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair or replace torn gaskets listed in the observation.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed a build-up of ice in the interior of the walk-in freezer at the time of inspection.
To prevent health hazards, equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair walk-in freezer and ensure food is not stored under leaking lines.
3717-1-04.4(Q) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Observed wash cycle thermometer not kept in good repair (dial not moving) at the time of inspection.
To ensure proper warewashing, (3) Ambient air temperature, water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use. Repair or replace the wash cycle temperature measuring device.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed accumulation of soil residue on nonfood-contact surfaces of the shelving in the walk-in units at the time of inspection.
To prevent contamination, Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Clean and maintain the items listed in the observation.
3717-1-04.6(A) / Food-contact surfaces and utensils - sanitization required
Observed equipment food-contact surfaces and utensils at the bar were not being sanitized at the time of inspection. Chemical sanitizer was measured below the minimum ppm according to the sanitizer label at the time of inspection.
To prevent health hazards, equipment food-contact surfaces and utensils shall be properly sanitized. Ensure test strips are being used to ensure proper concentration.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed single-service and single-use articles (boxes of cup, lids, etc.) were not properly stored in the dry storage room at the time of inspection. Boxes were found stored on the ground.
To prevent health hazards, single-service articles, and single-use articles shall be properly stored as described in this rule. (1) Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be stored: (a) In a clean, dry location; (b) Where they are not exposed to splash, dust, or other contamination; and (c) At least six inches (fifteen centimeters) above the floor unless the items are kept in closed packages on dollies, pallets, racks, and skids that are designed as specified under paragraph (II) of rule 3717-1-04.1 of this code;
3717-1-04.8(G) / Kitchenware and tableware - handling.
Repeat Observed single-service articles (drinking straws at bar) were not handled, displayed, or dispensed properly at the time of inspection.
To prevent contamination, single-service articles or single-use articles shall be handled, displayed, and dispensed so that contamination of food-contact-and lip-contact surfaces is prevented as described in this rule. Provide straws that are either wrapped or a dispenser that provides proper protection.
3717-1-05.1(Q) / Scheduling inspection and service for a water system device.
Observed water treatment devices for the ice machine were not inspected or serviced as required at the time of inspection.
To prevent health hazards, a device such as a water treatment device or backflow preventer shall be inspected and serviced in accordance with manufacturer's instructions and as necessary to prevent device failure, and records demonstrating inspection and service shall be maintained by the person in charge. Water filters shall be serviced according to the manufacturer.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of grease on the wall, floor and hood system around and above the fryers at the time of inspection.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean and maintain the area around the fryers.
