[?] A summary of the violations found during the inspection are listed below.

3717-1-04.8(E)(3) / Single-service articles and single-use articles - storage.
Single-service and single-use articles not protected from contamination. Observed plastric lids, bowls, containers on the shelving behind the front counter removed from the original packaging.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed damaged gasket for the tall reach-in refrigeration unit. Gaskets shall be cleaned as often as needed to be clean to sight and touch.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Damaged floor tile near the main hand sink. Flooring that is no longer smooth and easily cleanable shall be replaced or repaired.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Observed around the grease trap and the door frame leading into the warewashing room not properly sealed. Ensure any holes in the walls are filled with caulk, to be smooth and easily cleanable.

Comments
The operation was not operating at the time of inspection. The facility has been closed due to the COVID-19 pandemic.
Observed a replacement commercial chest freezer (Kelvinator KCNF073WS), old chest freezer has been removed.
For food defense, the equipment and products in room 201 shall be relocated to room 214.
The aerators on the plumbing fixtures throughout the facility shall be cleaned and ensure the water lines are flushed before using.
Ensure the clean-up kit and procedures are readily available and employees are trained to use it and know where to find it.
The violations documented shall be corrected and will be checked during next standard inspection.
